INVITATION TO TENDER /REQUEST FOR QUOTATION
1.0 INTRODUCTION
In compliance with the requirements of the Public Procurement Act,2007, the Federal Medical Centre, Yenogoa, Bayelsa State hereby invite interested, competent and reputable contractors/consultants with relevant experience, requisite expertise and good track record to submit Technical/financial Bids on the Project proposed to be implemented in the year 2017 National Health Insurance Scheme (NHIS) Intervention Programme (LOT 1) and Request for Quotation (Technical & Financial) for Oxygen Plant Services on Public Private Partnership (PPP) basis (LOT2).
2.0 PROJECT DESCRIPTION
- Lot 1: PROPOSED OF OXYGEN/REMODELING OF THEATRE COMPLEX 2 (NHIS Intervention Programme)
- Lot 2: PROVISION OF OXYGEN GAS PLANT SERVICES( Public Private partnership, PPP)
3.0 SCOPE OF WORK
- Expansion/ Remodeling of Theatre Complex 2(One Storey)
- To build, maintain and run the oxygen Plant on Public Private Partnership (PPP) basis in line with the terms and condition to be agreed upon.
- ELIGIBILITY CRITERIA
Interested contractors/consultants are required to submit the following document
- Evidence of registration with Corporate Affairs Commission (CAC), by submission of Certificate of Incorporation and Article of Association.
- Comprehensive company profile showing organization structure, list of staff including Curriculum Vitae of key personnel(enclose photocopies of professional qualifications)
- Evidence of Corporate Registration with relevant Professional Bodies as at 2017
- Evidence of Tax Clearance Certificate for the last (3) three years.
- Company Audited Account for the last (3) Years
- Evidence of compliance Industrial Training Fund Amendment Act, 2011 by submission of ITF Compliance Certificate for 2017
- Evidence of compliance with the Pension Reform Act 2014 by submission of current Pension Compliance Certificate for 2017
- Evidence of compliance with Nigeria Social Insurance Trust fund (NSITF) Act, 1993 by submission of NSITF Compliance Certificate for 2017.
- Evidence of Registration with the Bureau of Public Procurement (BPP) Database ( by proof of Interim Registration Certificate)
- Evidence of Financial Capability and Bank Statement of Accounts for the last 12 months.
- Evidence of performance of similar job completed for the last five years or on-going, stating clients, location, Letter of Award, Certificate of Completion and Contract Agreement/payment.
- A sworn affidavit confirming that:
- That all information presented and attached are true and correct of the company.
- None of the Directors have been convicted in any country for any criminal offence including fraud or financial impropriety
- None of the officers of FMC, Yenogoa or BPP is a former or present Director/staff of the company or has interest in the bidding
- Any other information that will enhance the application
- COLLECTION OF TENDER DOCUMENTS
Tender Documents are obtainable from the Procurement Unit, Prof, E. N. Etebu Administration Block, and Federal Medical Centre. Yenogoa within the working hours of 8am – 4pm (Monday to Fridays) upon presentation of evidence of payment of non –refundable Tender fee of N20, 000 for LOT 1
Payment should be made to Federal Medical Centre, Yenogoa through TSA Account REMITA Platform at any Commercial Bank.
6.0 SUBMISSION/CLOSING DATE OF TENDER DOCUMENTS
Completed Technical and Financial Bids(for both projects) should be submitted in two (2) separate envelopes and labeled “ Technical Bid” and “ Financial Bid” respectively with the company name indicated at the back of the envelope. The two envelopes should be enclosed in one bid sealed envelope and marked (Technical and financial), clearly stating the project name at the top right hand corner of the envelope and addressed to:

The document should be deposited in the Tender Box at the Procurement Unit, Prof. E. N. Etebu Administration Block, Federal Medical Centre, Yenogoa within 8am – 4pm of working days (Monday – Friday)
The deadline for submission is on Thursday 14th December, 2017 by 12 noon
7.0 OPENING DATE OF TENDER DOCUMENTS
Bid opening shall commence immediately after the closing deadline for submission of Bids in the Boardroom of Federal Medical Centre, Yenogoa on Thursday 14th December, 2017 by 12 noon.
All applicants or their representatives, relevant professional bodies and NGOs are invited to witness the opening exercise.
8.0 PLEASE NOTE THAT:
- Submission of Tender does not mean that Federal Medical Centre, Yenogoa is under any obligation whatsoever to award your company any contract.
- All required documents may be referred to the Security Agencies, Corporate Affairs Commission, Federal Inland Revenue Services (FIRS), PENCOM, ITF and other relevant bodies for verification
iii. Federal Medical Centre, Yenogoa reserves the right to annul the selection process at any time without incurring any liability and assigning any reason thereof.
- All bid submission must be in English Language.
- Application received after the deadline for submission shall be disregarded
