INVITATION FOR PRE-QUALIFICATION OF CONTRACTORS/SUPPLIERS FOR THE EXECUTION OF 2017 CAPITAL/NATIONAL HEALTH INSURANCE SCHEME (NHIS) INTERVENTION PROJECTS
1.0 INTRODUCTION
The Jos University Teaching Hospital hereby invites reputable and interested Contractors, Suppliers and Manufacturers with relevant experience and good track record of performance, to submit pre-qualification bids for the following jobs.
2.0 PROFILE OF PROJECTS
2.1 LOT A: CONSTRUCTION
i. Additional Wards
ii. Psychiatric Complex
2.2 LOT B: SUPPLIES
i. Medical/Hospital Equipment
ii. Supply of Anti Snake Venom
iii. Supply of Linens
iv. Supply of Office Furniture
2.3 LOT C: MEDICAL CONSULTANCY ON MEDICAL OUTREACH
i. Medical Outreach in Bauchi State
ii. Medical Outreach in Plateau State
2.4 LOT D: NATIONAL HEALTH INSURANCE SCHEME INTERVENTION PROJECT
i. Construction of General Out Patient Department (GOPD) Complex
ii. Medical/Hospital Equipment
iii. Furnishing
3.0 PRE-QUALIFICATION REQUIREMENTS
The following are the requirements that should be submitted by interested Contractors, Suppliers and Manufacturers on or before the closing date:
i. Evidence of company’s registration with Corporate Affairs Commission (CAC) by inclusion of Certificate of Incorporation and Article of Association.
ii. Evidence of Tax Clearance Certificate for the last three (3) years (i.e. 2014, 2015 & 2016) duly stamped corresponding with declared annual turn-over by inclusion of photocopy.
iii. Company’s Audited Accounts for the last three (3) years (i.e. 2014, 2015 & 2016).
iv. Compliance with the Industrial Training Fund (ITF) Amendment Act 2011 by inclusion of current compliance certificate from ITF.
v. Compliance with Pension Reform Act 2004 by inclusion of the current Clearance Certificate from the National Pension Commission (PENCOM).
vi. Submission of Interim Registration Report (IRR) issued by the Bureau of Public Procurement (BPP) as evidence of registration on the National Database of Federal Contractors and Service Providers.
vii. Evidence of compliance with the National Social Insurance Trust Fund (NSITF) Act by inclusion of current NSITF clearance certificate.
viii. Evidence of similar and verifiable projects successfully executed within the last three (3) years by inclusion of completion Certificates/Certificate for making good defects.
ix. Evidence of Financial Capability/Reference letter from a reputable bank (including demonstration of having Liquidity and/or evidence of access to availability of confirmed Credit facilities from a reputable bank.
x. Evidence of Registration with relevant professional bodies.
xi. Detailed Company Profile including Organizational structure, Curriculum Vitae of key personnel and photocopies of credentials.
xii. Sworn Affidavit declaring whether or not any officer of the relevant committees of Jos University Teaching Hospital (JUTH) or Bureau of Public Procurement (BPP) is a former or present Director, Shareholder or has a pecuniary interest in the prequalification and confirm that all information presented are true and correct in all particulars that the company is not in receivership or is the subject of any type of insolvency or bankrupt.
4.0 IMPORTANT INFORMATION
i. Only Pre-qualified companies shall be invited to competitively tender for the projects.
ii. Original documents on any aspects of a Company’s submission may be requested for sighting.
iii. The advert of “Invitation to Pre-qualification for 2017 Capital/NHIS Intervention Projects” shall not be constructed as commitment on the part of Jos University Teaching Hospital nor shall it entitle any company to make claims or seek any indemnity from the Hospital by virtue of having responded to the advertisement.
iv. Jos University Teaching Hospital reserves the right to reject/accept any/all bid documents without assigning any reason whatsoever.
v. Late submission will not be accepted.
vi. Notwithstanding the submission of Pre-qualification document, Jos University Teaching Hospital is neither committed nor obliged to include any Company on any bid list.
vii. Jos University Teaching Hospital reserves the right to verify any claim by companies. Companies shall therefore include necessary authorities and guarantee for such verification.
5.0 SUBMISSION OF PRE-QUALIFICATION DOCUMENT
The completed Pre-qualification document with full paginated should be in five (5) spiral bound copies (unbound submission will NOT be processed) and enclosed in an envelope, sealed and marked “PRE-QUALIFICATION FOR EXECUTION OF 2017 CAPITAL/NHIS INTERVENTION PROJECTS” for (Indicate the Lot) at the Top right corner of the envelope and addressed to:

Plateau State.
NOTE: The complete Pre-qualification document should be deposited in the Tender Box in the office of the Director of Administration, Administration Block, Jos University Teaching Hospital.
6.0 CLOSING DATE/BID OPENING
The submission of bids shall close on 12:00 noon Monday, 30th October, 2017. The document shall be opened by 12:00 noon, same day (Monday, 30th October, 2017) in the Boardroom of the Hospital at the Administrative Block.
The Representatives of Companies are invited to witness the Bid Opening at 12:00 noon on Monday, 30th October, 2017 in the Boardroom of the Hospital at the Administrative Block.
