Africa Field Epidemiology Network (AFENET) is the organization for applied epidemiology training programs in Africa. It oversees the Field Epidemiology (and Laboratory) Training Programs (FETP) in over 30 countries in Africa. It has vested with the responsibility of implementing Lagos AIDS indicator Survey (LAGAIS). In view of this, AFENET requires prospective bidders to submit quotations for the under listed Laboratory consumables for the fields work in the 20 Local Government Areas of Lagos State.

·         A copy of your Corporate Affairs Commission (CAC) certificate as proof of being a registered company for local vendors or duly registration certificate/license

·         Company Profile

·         Proof  of registration with NAFDAC (if applicable)

·         Letter of Authorization to distribute from supplier

·         VAT clearance certificate for the last 3 year;

·         A list of your past references; at least 3
