BACKGROUND

With funding from the Global Fund, Family Health International (FHI 360) is collaborating with Achieving

Health Nigeria Initiative (AHNI) to support the government of Nigeria to enhance sustained cross-sectional  integration of HIV/AIDS services by building Nigeria’s capacity to deliver sustainable high quality, comprehensive, prevention, treatment, care and related services in Nigeria.

Achieving Health Nigeria Initiative (AHNI) inviting qualified and interested companies to submit sealed quotations for the supply of the 136 units each Computer Desktop and Accessories (UPS) to strengthen Electronic Medical Record and District Health Information System at GF supported facilities.

SELECTION CRITERIA (Skill, Abilities and Qualifications)

1. Company Profile; Registered address, functional email and telephone numbers and point of contact for the company;
2. Evidence of legal registration document of the company (including CAC: FORM CO7, FORM CO2 etc);
3. Tax Clearance Certificate for 2014, 2015 and 2016
4. Approved Partnership, Dealership or distributorship certificate as applicable
5. Evidence of Financial Capability (Audited Accounts for the last 3 years and Bank Reference Letter)
6. Evidence of possession of experience in at least 3 similar jobs: Bidders must provide copy(ies) of Job Orders and proof of delivery
7. Reference letter(s) from an organization of similar award in size, scope and complexity within the last 3 years attesting to Satisfactory Performance
8. Cost
9. Warranty & Quality
10. Lead time
11. Payment Terms
12. Quotation Validity

SUBMISSION PROCEDURE AND INSTRUCTIONS:

Financial (Quotation) and other required commercial conditions (criteria 8,9,10,11 & 12) MUST be separated, signed by the authorized representative and in sealed envelope from the Technical Bid (Criteria 1-7 above).

Your submission must be in titled “RFQ/AHNi013/2017 – Procurement of Desktop Computers to support EMR and DHIS.

The completed RFQ/Tender document must be submitted to the address: “The Procurement Unit, FHI360 Country Office, Plot 1073 J.S. Tarka Street, Garki, Area 3 P.M.B. 44, Abuja, Nigeria by 2:00pm, Tuesday 19^thDecember 2017.

 This procurement is funded by the Global Fund and is subjected to the applicable rules and regulations of Global Funded awards. AHNI is Value Added Tax (VAT) exempted, but charges WHT (Withholding Tax) in compliance with relevant Nigeria TAX Laws.

 RFQ COMPANY’S GUARANTEE

All information submitted in connection with this Tender notice is valid for 60days from the RFQ due date. This include but not limited to cost, pricing, terms and conditions, service level and all other information.

AHNi reserves the right to select and negotiate with those company it deems in its sole discretion to be qualified for competitive bid and to terminate negotiation without incurring any liability. Issuance of Bid Document does not constitute a commitment by AHNi to award a contract.
