INVITATION TO TENDER/REQUEST FOR EXPRESSION OF INTEREST FOR THE EXECUTION OF 2017 CAPITAL/ZONAL INTERVENTION AND CONSTITUENCY PROJECT APPROPRIATION/NATIONAL HEALTH INSURANCE SCHEME (NHIS) INTERVENTION PROJECTS
1.0 PREAMBLE
Abubakar Tafawa Balewa University Teaching Hospital, Bauchi hereby invites competent, reputable and interested companies with relevant experience and good track records for the execution of its 2017 Capital Projects/Zonal Intervention and Constituency Projects/National Health Insurance Scheme Intervention Projects under the following:

2.0 ELIGIBILITY REQUIREMENTS
Interested Companies shall be required to submit the following documents:
a) Evidence of registration with Corporate Affairs Commission (CAC) by inclusion of Certificate of Incorporation and Article of Association.
b) Evidence of registration with Professional bodies such as ARCON, COREN, SURCON, CORBON, etc.
c) Evidence of current Tax Clearance Certificate for 3 years (2015, 2016 & 2017) corresponding with declared Turnovers by inclusion of photocopies.
d) VAT registration Certificate and remittance.
e) Company Audited Accounts for last 3 years (2015, 2016 & 2017) duly stamped by a reputable audit firm.
f) Evidence of Compliance with Pension Reform Act 2004 by inclusion of compliance letter from National Pension Commission expiring 2017.
g) Evidence of Compliance with Industrial Training Fund Amendment Act 2011 by inclusion of compliance letter from Industrial Training Fund (ITF) expiring 2017.
h) Evidence of registration with the Bureau of Public Procurement (BPP) National Database of Contractors, Consultants and Service Providers by inclusion of Interim Registration Report (IRR) expiring 2017.
i) Detailed Company Profile with list of Staff including Curriculum Vitae, Competence Qualifications and registration with relevant professional bodies and experience of key personnel.
j) Possession of Evidence of Ownership or Lease Agreement for relevant equipment, machines, and tools.
k) Evidence of financial capability /reference letter from a reputable bank (including demonstration of having liquidity and or evidence of access to or availability of confirmed credit facilities not less than N25Million from a reputable bank.
l) Evidence of experience in at least five jobs of similar nature and complexity successfully executed within the last five years with verifiable letters of award and certificate of job completion.
m) Evidence of compliance with National Social Health Insurance Trust Fund by inclusion of letter issued by NSITF.
n) Sworn affidavit indicating that all the information provided above is not false, fake or forged and that
1) None of the Directors of the company has been convicted in any Court of Law for Criminal offence involving financial impropriety.
2) No officer of the Abubakar Tafawa Balewa University Teaching Hospital, Bauchi is a former or present Director, Shareholder or has any pecuniary interest in the company.
3) The Company is not in receivership, insolvent or bankrupt.
4) Any other information that will further enhance the chance of shortlisting the company.
3.0 COLLECTION OF TENDER DOCUMENTS
Tender documents are to be collected from the Head of Procurement Unit old Administrative Building, Office No.7 Abubakar Tafawa Balewa University Teaching Hospital, Bauchi between the hours of 8:00am and 4:00pm daily Monday to Fridays upon presentation of a non-refundable cash payment tender receipt of N20, 000.00 (Twenty Thousand Naira) only obtained at Cash Office for each lot.
4.0 SUBMISSION OF TECHNICAL AND FINANCIAL BIDS/ EXPRESSION OF INTEREST
The prospective bidders shall enclose the Technical and Financial bids/ Expression of Interest in separate packaged sealed envelope clearly marked as either “Financial or Technical” tender as appropriate and put in a single (bigger) envelope neatly marked as “Technical /Financial bid/Consultancy Name for the Year 2017 Capital Appropriation Project/NHIS Intervention” indicating in each case the project Lot at the top left hand corner of each envelope addressed to: The Chief Medical Director, Abubakar Tafawa Balewa University Teaching Hospital off Yandoka Road, Bauchi. P.M.B. 0117, Bauchi, Bauchi State with their company names and contact numbers written boldly on it and dropped in the temper proof tender box located at the Boardroom, New Administrative Building, of Abubakar Tafawa Balewa University Teaching Hospital, Bauchi, between the hours of 8:00am to 4pm daily on or before 12:00 noon of Monday 27th November, 2017.
5.0 CLOSING DATE
Submission of tender closes at 12 noon, 27th November, 2017.
6.0 OPENING OF TECHNICAL AND FINANCIAL BIDS
Technical bids shall be publicly opened immediately at the expiration of the deadline for submission on 27th November, 2017 in the presence of representatives of responded companies, Non-Governmental Organizations (NGOs), civil societies, and the general public.
7.0 OTHER INFORMATION
• Late submission after the deadline shall be rejected.
• Failure to comply with instructions and provide any required documents may automatically lead to disqualification.
• Consultants and Contractors are allowed to make expression of interest /bid for more than one lot.
• Only shortlisted consultancy firms will be invited for further consideration.
• All required documents may be referred to the Security Agencies, Corporate Affairs Commission, Federal Inland Revenue Service (FIRS), PENCOM, ITF and other relevant bodies for verification.
